Panama Canal Birth Certificates & Death Certificates
Anyone born in the U.S. Panama Canal Zone while the U.S. government administered the area from 1904 to the end of 1999 is considered a citizen of Panama.
Since Panama took control of the Panama Canal Zone on Noon December 31, 1999 all of the records pertaining to births and deaths in the U.S. Panama Canal Zone are stored with the U.S. State Department.
According to the U.S. State Department, only the following people can request a copy of a Panama Canal Birth Certificate:
- The individual named on the birth certificate who is 18 years or older
- A parent or legal guardian of the person listed on the birth certificate
- A government agency which is authorized
- An individual having written authorization
The fee is $50 for every Consular Vital Record document.
To obtain a Copy of a Birth Certificate issued in Panama Canal Zone the following documents must be submitted:
1. A signed hand written or typed request (notarized) including the following information:
- a) Full name of the child when born
b) Adoptive name of the child (if applicable)
c) Place and date of birth
d) Full name of both parents, their place of birth and date
e) U.S. passport information of the child
2. Copy of applicant’s legal ID (driver’s license, passport, state ID)
3. A $50 money order or check for each document requested payable to “Department of State”.
4. Send by Mail or Courier to:

To Obtain a Copy of a Death Certificate issued in the Panama Canal Zone, Submit:
1. A notarized hand written (or typed) request including the following information:
- a) Full name of deceased
b) Place and date of death
c) Applicant’s signature
2. A copy of applicant’s legal ID
3. $50 money order or check for each requested
copy payable to “Department of State”.
4. Send by Mail or Courier to:

Overnight Delivery
You can send the application using an overnight courier service. To be returned by overnight courier or mail service, add an additional $12.72 with the application or enclose a pre-paid envelope using any courier service.
